I’m Dr. Stephanie Dunlop, a board-certified emergency physician. For years I said those words to women in my ER, because that is what the panel in front of me showed.

Then it happened to me. After residency and a new baby, I was exhausted in a way sleep didn’t touch, gaining weight without changing anything, and losing words mid-sentence. I ran my own labs. TSH: 4.2. Normal by the reference range.

I felt anything but normal.

That number sent me into the research on optimal ranges, and what I found changed how I practice medicine. The system I trained in waits for disease. It has no answer for the years of dysfunction that come first, when a woman knows something is wrong and every test says she is fine. Why “normal” fails you

Normal is an average, never a target

A “normal” lab range covers roughly the middle 95 percent of the tested population. That population includes the chronically stressed, the undiagnosed, and the unwell. Land anywhere inside it and you are called fine.

Optimal is a different question. Take TSH, the standard thyroid screen. Most labs flag it above 4.5. Most women feel their best between 1.0 and 1.5. The space between those numbers is where millions of women live for years: symptomatic, dismissed, and told they are fine.
